HOW DO I ARRANGE MY CARE WITH YOU?

Deciding that you need support is an incredibly big step for you and your loved ones.  You can ring us directly on 01634 853187 where we can have a chat about what you would like from us, email us at info@communitycareline.uk or complete the enquiry form in the “Contact Us” section on this web site and we will get back to you as soon as we can and answer all of your questions.

MY NEEDS HAVE CHANGED, WHAT CAN I DO?

Your care plan is completely about you, your needs, goals you may have and we understand your needs may change.  Regular reviews are completed with you, but you can tell us at any time if you feel that your care package needs updating.  A member of the team will come and visit you and put together your new plan.

DOES MY CARE WORKER KNOW WHAT I WOULD LIKE THEM TO DO?

​Community Careline Services (Medway) Ltd ensure that your care package focuses on your individual needs and preferences.  Care Workers will have access to the care plan that you have agreed to vis the app they use.  Their training tells them that they must adhere to your planned care and if there are changes that are needed they will inform the Senior Care Workers.

IS THERE ANYTHING MY CARE WORK IS NOT ALLOWED DO?

Care Workers cannot and will not make decisions for you or tell you what you need or what needs to be done.  Everything that you have agreed in your plan of care is what they will help you with.  During your initial assessment you will be advised if there is something that cannot be done by one of our Care Workers and alternatives can be offered.

WILL I SEE THE SAME CARE WORKER REGULARLY?

Building trusted relationships are a vital part to delivering you the care that will make a difference to you.  We will do our very best to deliver continuity in your care which means having a regular set of Care Workers.  

HOW DO YOU PROTECT MY PERSONAL INFORMATION?

Community Careline Services (Medway) Ltd adheres to GDPR regulations and the NHS Data Protection Toolkit, your personal information is kept securely and only those who need this information will have access to it.  All our systems meet GDPR requirements when it comes to security.

HOW DO MY LOVED ONES AND FAMILY KNOW WHAT HAS HAPPENED IN MY CARE VISIT?

Technology plays a large part in what we do at Community Careline Services (Medway) Ltd.  The system we use has a friends and family portal that is designed to let your loved ones and family see when your care visit has taken place, what the Care Worker did and any other information that the Care Worker thinks is important on their visit.  You can can be invited to log into this platform by providing your email address with consent from the client themselves.
